In preparation for your first appointment with our office, please download and fill out our NEW PATIENT INTAKE FORM, and bring them with you to your first scheduled visit.

We suggest that you arrive 10-15 minutes before the start of your first scheduled appointment to complete our registration process. Please bring:

  • Forms you completed at home
  • Therapy Prescription (if you have one)
  • Insurance Card / Case Number
  • Any relevant diagnostic reports you may have (for example: x-ray or MRI)
  • Wear comfortable clothes that will allow the Physical Therapist access to the body part for which you are being seen

When you arrive we will begin with an Initial Evaluation where we will review your medical history and your current symptoms. Be prepared to answer questions such as:

  • What types of symptoms are you experiencing?
  • How long have you been experiencing these symptoms?
  • How did it start?
  • What aggravates and eases these symptoms?

Our evaluation requires a holistic approach that involves questions regarding the patient's experience of pain in the muscles, bones and joints. Other important factors include weakness in extremities, limitations on range of motion and tendency toward fatigue. Please be sure to tell your Physical Therapist about any previous Therapy Treatment you may have had, as well as the results.

We will also do an Extensive Examination.  During your exam, your Physical Therapist may evaluate the following:

  • Strength
  • Range of Motion
  • Flexibility
  • Posture
  • Mobility and Movement Patterns

What To Expect

After your evaluation and examine, your Physical Therapist will explain his or her assessment of your condition and explain the Individual Treatment Plan that has been designed for you. Your Physical Therapist will also introduce you to the team that will be involved in your rehabilitative care.

If at any time you have questions regarding any aspect of your Treatment Plan, do not hesitate to ask. Your understanding of our Assessment and Treatment Plan is essential to helping you achieve your goals, and is fundamental to a successful outcome.

The next step of your initial appointment will be to begin your treatment, which will most likely involve the following:

  • Some Hands-on work
  • Therapeutic  Modalities, such as ice, heat, or electrical stimulation
  • A Therapeutic Exercise Program
  • Education about Activity Modification
  • Education about a Home Exercise Program, designed specifically for you

Since you will only spend two to three hours a week receiving in office treatment, what you do before and after treatment is very important.  Following through with your Home Exercise Program and Activity Modification is likely to help you attain your goals much faster.
